{"id":11165,"date":"2021-11-30T10:22:33","date_gmt":"2021-11-30T04:52:33","guid":{"rendered":"https:\/\/i3tk.org\/demo2\/?p=11165"},"modified":"2021-11-30T10:22:36","modified_gmt":"2021-11-30T04:52:36","slug":"two-patients-die-of-new-strain-of-fungus-in-aiims","status":"publish","type":"post","link":"https:\/\/i3tk.org\/demo2\/world-news\/resources\/two-patients-die-of-new-strain-of-fungus-in-aiims\/","title":{"rendered":"Two patients die of new strain of fungus in AIIMS"},"content":{"rendered":"\n<p>NEW DELHI: Doctors at the All Indian Institute of Medical Sciences have confirmed the<br>presence of a drug resistant pathogen called Aspergillus lentulus in two patients suffering<br>from chronic obstructive pulmonary disease (COPD) admitted to the hospital recently. Both<br>died during treatment.<\/p>\n\n\n\n<p>Aspergillus lentulus is a species of the Aspergillus fungus that is known to cause infection in<br>the lungs. It was first described in medical literature in 2005. Since then, several countries<br>have confirmed infections in humans. However, this is the first time that this species of<br>Aspergillus has been reported to have infected patients in India, according to doctors.<br>According to a case report published in Indian Journal of Medical Microbiology (IJMM), one<br>of the patients was in his late 50s, the other in his early 40s. Both had COPD. The first man<br>was referred to AIIMS from a private hospital when the infection did not subside despite<br>treatment, including the administration of antibiotics and antifungals, in addition to<br>supplemental oxygen therapy.<\/p>\n\n\n\n<p>The IJMM report said the patient was administered Amphotericin B and oral Voriconazole<br>injections at AIIMS. There was no improvement in his clinical condition for over a month<br>before he succumbed to the fungal infection.<\/p>\n\n\n\n<p>The second patient was rushed to the AIIMS emergency with fever, cough and shortness of<br>breath. He was also administered Amphotericin B, but there was no improvement in his<br>condition and he died after a week of multi-organ failure.<\/p>\n\n\n\n<p>\u201cTo the best of our knowledge, this is the first report from India of cases of invasive<br>pulmonary aspergillosis in a COPD patient caused by Aspergillus lentulus,\u201d states the IJMM<br>report authored by Jaweed Ahmed, Gagandeep Singh, Immaculata Xess and Mragnayani<br>Pandey from the microbiology department of AIIMS and Anant Mohan, Janya Sachdev,<br>Prashant Mani and Bhaskar Rama from the department of pulmonology there.<\/p>\n\n\n\n<p>There are over a million species of fungi in the environment. Dr Arunaloke Chakrabarti, who<br>heads the WHO Collaborating Centre for Reference and Research on Fungi of Medical<br>Importance, told TOI that till about a decade ago, 200-300 of them were known to cause<br>disease. \u201cBut now, there are more than 700 identified fungal species that are known to cause<br>diseases. Some of them are resistant to available drugs,\u201d he said.<\/p>\n\n\n\n<p>Chakrabarti, who heads the department of medical microbiology at PGI Chandigarh, said he<br>had been raising the issue of rising fungal infections and measures needed to prevent the<br>morbidity and mortality caused by it for 37 years, but it hadn\u2019t caught the attention of experts.<br>\u201cThen the Covid-19 pandemic occurred and mucormycosis \u2014 another fungal infection \u2014<br>emerged as a major cause of death in seriously ill Covid patients. That made policymakers<br>sit up and think about the importance of creating infrastructure for timely diagnosis and<br>management of fungal infections,\u201d said Chakrabarti.<\/p>\n\n\n\n<p>Fungal infections are a group of diseases caused by fungi, organisms found freely in the<br>environment, according to Dr Chand Wattal, chairperson, Institute of Clinical Microbiology<br>and Immunology, Sir Ganga Ram Hospital. He added that most infections caused by fungi<br>were treatable, for example, ringworm or nail infections. \u201cBut there are infections caused by<br>species of fungi such as Candida, Aspergillus and Mucor that can cause systemic illness in<br>persons with weaker immunity and co-morbidities like diabetes. The infection can easily<br>spread in the bloodstream and can get lodged in different parts of the body which is difficult<br>to eradicate,\u201d added Wattal.<\/p>\n\n\n\n<p>There are only three classes of antifungal drugs. Infection caused by Candida auris is<br>untreatable by most of them, which is another challenge. \u201cTill around 15 years ago, only 30%<br>of infections caused by Candida were drug resistant. Now, almost 70% of these infections<br>are drug resistant. This poses a serious problem in treating patients,\u201d said Dr Sumit Ray,<br>head of critical care at Holy Family Hospital. The high cost of antifungal drugs was another<br>major impediment in saving lives, he added.<\/p>\n\n\n\n<p>With increase in longevity as well as the incidence of co-morbidities such as diabetes and<br>chronic kidney disease, doctors say the incidence of life-threatening fungal infections was<br>likely to go up. \u201cOne of the major reasons for the rise in fungal infections is excessive use of<br>antibiotics in India. It kills both the bad and the good bacteria, thus creating favourable<br>conditions for the growth of funguses that are known to cause diseases,\u201d explained a senior<br>doctor.<\/p>\n\n\n\n<p>Overuse of steroids is another key factor. \u201cSteroids suppress the body\u2019s immune system,<br>thus reducing its ability to fight against fungal infections.
